Please check that you are up to date with the latest information in regards to requirements for travel with your pet to your destination country (including those you may be transiting through) and return to the UK. It is your responsibility, as the pet owner, to ensure all final documents meet requirements and the information provided to us is correct. Check the latest information on pet travel to Europe on Gov.uk.
Please complete the form below at least 72 hours prior to your appointment. Please make sure you tick the declaration at the end of this form, print and sign your name. Forms can also be emailed to us (email@example.com) or dropped off at the practice.
On the day of your appointment, we will need you to bring with you:
- proof of rabies vaccination (including historical vaccinations)
- proof of identification of your pet(s)
If you have a Pet Passport for your pet that was issued before 31/12/2020 this will suffice. If you do not have a Pet Passport for your pet then please bring your vaccination card or certificate that shows the current rabies vaccination, and a document confirming your pet has been microchipped.
